A 28-year-old Pregnant Woman with Rare Takayasu’s Arteritis Successfully Delivers a Healthy Baby

MUMBAI, 1st APRIL, 2023 (GPN): A team consisting of Dr. Rajashri Tayshete Bhasale, Consultant Gynaecologist & Obstetrician, Laparoscopic Surgeon; Dr. Mayuresh Pradhan, Consultant Cardiovascular & Thoracic Surgeon and Dr. Anup Taksande, Consultant Interventional Cardiologist, helped a 28-year-old woman with a rare Takayasu’s Arteritis to successfully deliver a healthy baby boy weighing 2.4 kgs.

Patient Sheetal Shah (Name Changed), a housewife and a resident of Bhayander, was excited about her first pregnancy. Her happiness was short-lived as 3 months into the pregnancy, she was detected with a rare condition called Takayasu Arteritis. She had constant high BP and also unequal BP pulse in both her arms. She was further diagnosed with Coarctation of Aorta and was advised against continuation of pregnancy as it was life threatening.

Dr. Rajashri Tayshete Bhasale, Consultant Gynaecologist & Obstetrician, Laparoscopic Surgeon Wockhardt Hospitals Mira Road, said, “Patient came in emergency at 9 months of pregnancy with BP in right arm 190/110 & left arm 120/90 she was known to have Takayasu Arteritis which is a large vessel inflammation disease. It is a group of disorders that cause blood vessel inflammation, mainly affecting the aorta. Despite being on anti-hypertension medication, her leg pulses were absent, we also call it pulseless disease. Her ECHO showed severe Coarctation of Aorta with Mitral Regurgitation.

Dr. Rajashri Tayshete added, “after, due counselling elective LSCS was done alongwith cardiac Anaesthesia team. Dr. Roopa Mepani and Dr Kedareshwar Pote said, we were successful in delivering a 2.4 kgs baby. Post surgery period, the patient was monitored in ICU was uneventful and delivery in this condition is always full of complications but the patient responded well. Complications are stroke, sudden death, cardiac failure etc. Patient recovered well & is now enjoying her motherhood.”

Dr Mayuresh Pradhan said, “She was a case of a rare disease called Aortoarteritis and she had a significant narrowing of the main artery which supplies the lower part of the body. As a result, the lower body gets reduced blood supply. Generally, patients are advised termination of pregnancy when having such a condition but this patient decided to continue the pregnancy.  Such patients require Caesarean Section and cannot have a normal vaginal delivery. The anaesthesia is challenging and requires monitoring of upper and lower limb pressure simultaneously in order to maintain good lower body perfusion. Such patients can land up in Heart failure once the baby is delivered. The surgery was uneventful and now the patient will require treatment for her narrowing of the main artery which in medical terms is called Coarctation of aorta.
